To access the application for Gallatin CNA School,

 click on the button below that is labeled “DOWNLOAD.” 

That will open the application form as a Word

document.

When you open the file, you can complete the information in several ways:

1. If you have access to a printer, you can print it out and mail a paper copy to the

address on the form.

2. If you do not have a printer, you can copy and paste the information requested on

the form into an email, and add your answers in that email. Send the email to the

email address on the form.

3. You also could type your answers directly on the form, which is a Word document.

Save your work in that document file, attach the file to an email, and send it to the

email address on the form.

4. You could convert the Word file to a pdf file. In the pdf file, if you double click on the

blank you want to complete, a text box should appear that will allow you to add your

information. Again, save your answers in that document, attach the file to an email,

and send it to the email address on the form.

5. Finally, you can take a photo of the entire completed form on your phone, and send

the photo in a text message to the cell phone number on the application form.
